To Enrol...

  1. Print and complete the (click on "enrolment form" link with the right mouse button and choose Save Target As. This will enable you to save the form to your PC)
  2. Post the completed form with enrolment fee cheque to:
    Lifesong

  3. Or to pay electronically, please make your payment to the following bank account
    Bank: ANZ Account Name:
    Lifesong BSB: 012330
    Acct #: 498173565
     and email the receipt of transfer details to km@molins.net.au
    Please list your child's name in the transfer details.

Reenrolling by email

If you are reenrolling and your details have not changed just email me your class preferences and you child's name together with payment details. I will confirm a place and receipt of payment by email before the class begins.

Or to Enrol by email...

  1. Dowload and read Lifesong's Kindermusik Policy (click on "Policy" link with the right mouse button and choose Save Target As. This will enable you to save the form to your PC)
  2. Copy, paste and complete the email enrolment questions from a Word document into your email message and send to:
    email address
  3. Please also email us the electronic transfer details and we will acknowledge receipt of payment once transfer has been processed.
